I attempted to log into my website using my administrator account this morning only to find that I had been blacklisted by the Bad Behavior plugin.
The Cause
It turns out that the creator of the Bad Behavior plugin caused an issue with the integration of a third-party system that managed the blacklist file. Thus it automatically blacklisted my account the next time I tried to login.
The Solution
Luckily this is extremely easy to fix. You simply need to download the latest copy of the Bad Behavior plugin, ftp the new files over top of the old files, and attempt to log back into your account.

Well, look like I also can’t replicate the error message. Look like some random Wordpress bug.
Here is what I know from the message:
1. Error: Duplicate entry when trying to insert to table wp_useronline
2. The post being recorded: "Wordpress Plugin Framework v0.04 Released"
3. The referrer URL: http://weblogtoolscollection.c.....tall-tool/
That’s correct, I was accessing your post through that WebLogToolsCollection post and did a duplicate entrance and saw the error. I tried that again now but seem like can’t. Look like the time of error somehow I generate 2 request at exactly the same microsecond.
It’s way off topic so sorry about that.

No Keith, this error appeared before I write any comment. It’s something to do with your plugin that access wp_useronline, which is not related to commenting. It should be something to do with WP-Stats plugin if you have, or some similar one.

Binh - I am not sure how you encountered this error. It looks like you may have tried to post the same comment twice. I will try to replicate this issue to see if I can find the source of the error.
